Does this item come with the spray nozzle?

No, the nozzle is a part along with the wiper motor assembly. Rear wiper Motor & Nozzle Thanks for your inquiry.

do you need any additional tools to install/replace this part? I have a 2011 Jeep Wrangler 4-door. thanks

A wiper arm removal tool may be needed (#92810.2088). This would depend on how well the original arm is still pressed onto the splines. In some cases the splines have been stripped and the arm will come right off.
